چکیده

Major purpose of the management of thyroid nodules is to distinguish between malignant nodules and the benign ones. Nodules with the cytology result Atypia of Undetermined Signifi cance or Follicular Lesion of Undetermined Signifi cance are still a diagnostic challenge. Initial presentation of the Bethesda System was based on 7% diagnosing rate and around 5-15% malignancy rate for these nodules; however, clinical practice presents with higher rates for both variables. Recent studies showed that when these nodules are grouped by cytological features; compared with the architectural atypia group, the cytological atypia group has a signifi cantly higher risk of malignancy. Cytological sub-classifi cation, ultrasonographic evaluation and genetic tests in selected cases should be used in the management of these nodules. Atypia of Undetermined Significance/Follicular Lesion of Undetermined Significance (AUS/FLUS): Interpretation and Algorithm for Follow-Up

The Bethesda System for Reporting Thyroid Cytology (TBSRTC) has proven to be an effective and robust thyroid fine needle aspiration (FNA) classification scheme to guide the clinical treatment of patients with thyroid nodules. However, a tendency of increasing diagnosis of atypia of undetermined significance/follicular lesion of undetermined significance (AUS/FLUS) is observed. This is commensur...

Benign Aspirates on Follow-Up FNA May Be Enough in Patients with Initial Atypia of Undetermined Significance/Follicular Lesion of Undetermined Significance

Background. Management of thyroid nodules with benign aspirates following atypia of undetermined significance/follicular lesion of undetermined significance (AUS/FLUS) is not well established. We reviewed the risk of malignancy and the role of ultrasound (US) features among thyroid nodules with benign results following initial AUS/FLUS diagnoses. Methods. From December 2009 to February 2011, a ...

Differential diagnosis of thyroid nodules using fine-needle aspiration cytology and oncogene mutation screening: are we ready?

Thyroid nodules are a very common clinical finding, and although the majority of them are benign, thyroid carcinoma accounts for about 5-15% of nodules. Fine-needle aspiration cytology (FNAC) is actually used for the differential diagnosis of these lesions. Although in most cases this examination clearly distinguishes benign from malignant lesions, some fine-needle aspiration (FNA) samples fall...

Role of Core Needle Biopsy in the Management of Atypia/Follicular Lesion of Undetermined Significance Thyroid Nodules: Comparison with Repeat Fine-Needle Aspiration in Subcategory Nodules.

BACKGROUND The role of repeat fine-needle aspiration (RFNA) or core needle biopsy (CNB) has not been established in nodules categorized as atypia/follicular lesion of undetermined significance (AUS/FLUS). OBJECTIVE The purpose of this study was to retrospectively determine whether CNB is more useful for management decisions than RFNA at each subcategory of AUS/FLUS nodules. METHODS This stu...

An independent study of a gene expression classifier (Afirma) in the evaluation of cytologically indeterminate thyroid nodules.

CONTEXT Molecular markers hold the promise of improved diagnostic yield in thyroid fine-needle biopsy. The Afirma gene expression classifier (GEC), available commercially, reports a negative predictive value of 94% in the diagnosis of benign nodules after indeterminate cytology. However, there are currently no independent studies of the performance of this assay. OBJECTIVE The aim was to asse...
